[2]Decoupling capacitors should connected control terminals (VCTR1, VCTR2) close possible. values these capacitors should selected from 100pF range. Please consider that these values very effective switching time (Larger capacitor gives longer switching time). [3]In order keep good isolation characteristics, ground terminal (5pin) should connected ground pattern with wider width close possible, through-hole ground plane should also placed close possible.
